Agreement to Sign for Insurance Electronically

Although I am aware of the various alternative methods for signing insurance documents, I freely choose to sign for insurance using the Wizard Lizard online EZBuy e-signature process instead of signing in-person at an insurance agent’s or broker’s office, instead of using regular mail, instead of using a fax machine or instead of using any other method. I understand and acknowledge that the EZBuy process does not transact insurance on anybody’s behalf and only provides a function of convenience that allows me to interact with an agent, broker or an insurance carrier remotely online for the purpose of signing needed paperwork for insurance. 

I understand and acknowledge that the EZBuy system will allow me to electronically provide my signature on signed documents so that an agent, broker or insurance carrier can submit and/or process an insurance application. I further understand that EZBuy does not activate insurance coverage nor confirms that insurance coverage has been successfully put into effect and that the activation of coverage and confirmation that my insurance is in effect can only come directly from the agent, broker or insurance carrier and does not come from EZBuy. I understand that I will become a formal client/customer of the agent, broker and/or insurance carrier who is part of this insurance application process and become a formal client/customer of Wizard Lizard LLC, the maker of EZBuy, once the EZBuy process has been successfully completed. 

I understand that in order to sign up for insurance online I will be reading forms, disclosures, applications, agreements and any other needed documents on the EZBuy website instead of reviewing these items on printed paper. I understand that in order to sign for insurance using the EZBuy e-signature process I will be clicking on items that may be a check box, a text space or an item that says "Signature", "Initials", "Date", "Accept" or "I Agree". I am aware that every time I click on a check box, a text space or an item that says "Signature", "Initials", "Date", "Accept" or "I Agree" I intend that click to be my "electronic signature" and to have the same effect as if I had actually handwritten a signature on a paper document or application. I also understand that my electronic signature(s) will be reflected in the resulting records as date and time stamps shown in an audit trail that will be "hashed" (locked down) when all required signatures have been completed. I understand that a copy of the documents that I have signed and the resulting audit trail will be made available to me at the end of the EZBuy session. I understand that I may stop or discontinue the EZBuy e-signature process any time prior to my initiating the final "click" and that signatures, agreements or obligations associated with this process will not be complete or in force between any of the parties to this EZBuy e-signature ceremony. 

Electronically Signing In-Person- I understand that I may electronically sign these documents in-person with the insurance agent, broker or a carrier representative. I understand that part of the e-signature process requires that I receive all notices and finished documents immediately after completing the signing process. I understand that those notices and finished documents are typically sent immediately to my email address or another known email address for my immediate retrieval at the completion of signing. I understand that if I elect to sign in-person and therefore unable to immediately retrieve these notices and finished documents from any email address, or I am limited to retrieving these notices and finished documents using the insurance agent’s, broker’s or carrier’s email address, I agree to accept printed hard copies of these notices and finished documents during my in-person visit that are handed to me by the insurance agent, broker or insurance carrier assisting me with the in-person electronic signing. I also agree to request these notices and finished documents from the insurance agent, broker or carrier if they are not given prior to my leaving the in-person signing visit. 

I agree to hold harmless Wizard Lizard LLC, its officers, directors, employees, agents, subsidiaries, affiliates, business partners, successors, assignees, heirs and survivors for the actions of the agent, broker or insurance carrier initiating this EZBuy ceremony or for any and all information, text, or errors contained in the online forms, disclosures, applications, agreements and any other documents used in EZBuy and for any and all damages resulting from an insurance loss or claim as well as adverse occurrences associated with internet connections, speed, compatibility, interruptions, viruses, software, hardware, systems, loss of data, unauthorized users, lost/stolen user ID, passwords along with any action taken against the insurance agent, broker or insurance carrier associated with this insurance purchase by any governmental agency, regulatory body, insurance customer or other insurance carrier. I agree to take full responsibility for the accuracy and effectiveness of all information that I have provided and subsequently used by the agent, broker or insurance carrier in conjunction with this EZBuy e-signature ceremony. 

I have freely and independently made this decision to sign for insurance using the EZBuy e-signature process at my sole discretion and I have not been influenced in my decision to do so by Wizard Lizard LLC, the insurance agent, broker, insurance carrier or any of their representatives.

Consent to Receive Documents Electronically

As a part of every insurance application, you are entitled to receive copies of certain documents, notices and disclosures.Usually, those documents, notices and disclosures are on pieces of paper that are handed or mailed to you by your insurance agent or broker when you apply for insurance. You may choose to use the Wizard Lizard online EZBuy e-signature process as analternative way to sign insurance documents, pay the initial down payment and receive copies of documents that you signedrather than going in person to an agent’s or broker’s office, using regular mail, using a fax machine or other methods. However you must agree that these documents, notices and disclosures can be given to you through the internet during and after the completion of the EZBuy online process.  If you change your mind at any time during the EZBuy process, you may simply close your Internet session, go to the agent’s or broker's office and apply for your insurance in person. Your consent to receive these documents, notices and disclosures via EZBuy over the Internet applies only to this one online insurance e-signature session. If you want a paper copy of these documents, notices and disclosures from this session, you may print them on the printer attached to your computer at the end of the session. If you want to see a copy of these documents, notices and disclosures or any other portion of your EZBuy e-signature ceremony after completion of this online session, simply click on the link shown on the EZBuy Receipt that you will receive by email to access the login or by going to www.ezbuyesales.com to login. Once logged in, type in the email address and zip code that you used to for this EZBuy session and you will be able to review and print the various pages completed through EZBuy. All of your documents, notices and disclosures completed through EZBuy will remain available for the statutory time requirements dictated by the department of insurance in your state. You can also request that the agent or broker give you a paper copy of these documents by contacting the agent or broker directly.

To use the EZBuy online e-signature process, you will need a computer connected to the Internet with the following minimum specifications:

-Microsoft Internet Explorer version 6 or higher/compatible with 128 bit encryption
-Safari version 1.2 or higher/compatible with 128 bit encryption

If you can view this page, your computer equipment is sufficient to use the EZBuy online e-signature process and receive any required documents, notices and disclosures.

THE WIZARD LIZARD / EZBuy PRIVACY POLICY
EZBuy is a web based process developed by Wizard Lizard LLC that is used for gathering and conveying online the personal information and electronic signatures needed to produce and complete an insurance application between an insurance agent or broker and the customer. EZBuy has physical, electronic, and procedural security measures in place designed to protect your personal information. The only employees who have access to your information are those with a need to know to serve and accommodate the EZBuy process. 

Information Collected Through EZBuy
The following personal information may be collected, verified and transmitted through the EZBuy process between you and the insurance agent, broker, insurance carrier, insurance GA/MGA or third party insurance processing and driving/vehicle record vendor via the internet for the purpose of completing an insurance application:
Personal Information may include any identifying information, such as:
-Your name, names of others, address, e-mail address, phone number;
-Age, marital status, occupation, driver's license number, social security number, assets, income, and beneficiaries;
-Claim history, driving record, car make and model, license plate number, lender and leasing agent;
-Credit card, debit card, checking account, credit history, credit standing, policy coverage, premiums, and payment history. No credit card, debit card or checking account information is store or retained by Wizard Lizard or EZBuy.

EZBuy conveys personal information provided by:
-You or your respective insurance agent or broker;
-Documentation you complete and submit through EZBuy such as disclosures, e-signatures, applications or portions of applications and forms;
-Consumer reporting agencies;
-The DMV to verify driving record, auto title, or license status;
-Claims databases, such as the Comprehensive Loss Underwriting Exchange (CLUE).

How EZBuy Uses and Discloses Personal Information
In order for the EZBuy process to facilitate the completion of an insurance application, your personal information will be securely conveyed back and forth via the internet between you and the insurance agent or broker that is assisting you with your insurance transaction. Your insurance agent or broker will have custody of your personal information as well and it is recommended that you review the agent’s or broker’s Privacy Policy. EZBuy does not sell or provide personal information or customer lists to any other person(s) or entity for any reason other than those stated here. Your collected information may occasionally be used to notify you of products or services that might be of interest to you. If you do not wish to receive product or service information, simply send us an email stating 'no information' at sales@ezbuyesales.com. 

Information transmitted through EZBuy may be revealed to:
-Financial companies such as life, medical, automobile, and property insurers, premium finance companies, insurance Managing General Agents and General Agents and Surplus Lines Brokers;
-Service providers that help process or service your insurance account and payment;
-Medical-care facilities or providers to verify coverage and benefits;
-A court, governmental or regulatory agency when there is a lawful request;
-Law enforcement when lawfully requested to protect against fraud or unauthorized transactions;
-A group policyholder for purposes of reporting claims experience;
-Our legal counsel for review and opinion; and
- As otherwise permitted or required by law.

Use of Cookies
